You may access the survey using your NYCSA (New York City Schools Account) or at this link: https://surveys.panoramaed.com/nycdoe/login

You will need your child’s Student Identification number (OSIS) which is found on their report card or NYCSA account. After you enter the identification number you will enter a lowercase f. (For example: 987654321f ).

After you complete your survey, send a screenshot of the confirmation to the PTA email account and your name will be entered into a raffle to receive gift cards! The PTA email is: 32k383pa@schools.nyc.gov. Thank you!
